ABSTRACT

Gas therapy has been proven to be a promising and advantageous treatment option for cancers. Studies have shown that nitric oxide (NO) is one of the smallest structurally significant gas molecules with great potential to suppress cancer. However, there is controversy and concern about its use as it exhibits the opposite physiological effects based on its levels in the tumor. Therefore, the anti-cancer mechanism of NO is the key to cancer treatment, and rationally designed NO delivery systems are crucial to the success of NO biomedical applications. This review summarizes the endogenous production of NO, its physiological mechanisms of action, the application of NO in cancer treatment, and nano-delivery systems for delivering NO donors. Moreover, it briefly reviews challenges in delivering NO from different nanoparticles and the issues associated with its combination treatment strategies. The advantages and challenges of various NO delivery platforms are recapitulated for possible transformation into clinical applications.

ABSTRACT

Objective:To evaluate the efficacy and safety of of rivaroxaban for different doses in the treatment of isolated distal deep vein thrombosis.Methods:The clinical data of 853 patients of isolated distal deep vein thrombosis attending Nanjing Drum Tower Hospital from Jan 2018 to Dec 2020 was retrospectively analyzed.Results:Thrombotic recurrence rate increased with increasing follow-up in the standard and low dose groups, and it was significantly lower in the standard dose group than in the low dose group (HR=0.44, 95% CI: 0.25-0.78, P=0.005) with most thrombosis occurring within the first year of follow-up. There was no statistical difference between the two groups in terms of major bleeding events (HR=1.70,95%CI 0.56-5.14, P=0.530) and the incidence of clinically relevant non-major bleeding events was significantly higher in the standard dose group than in the low dose group (HR=2.36, 95%CI 1.26-4.44, P=0.020). Subgroup analysis on anticoagulation duration found when anticoagulation duration was longer than 1.5 months, the risk of thrombosis was lower in the standard dose group than the low dose group (1.5-3 months:HR=0.11, 95%CI 0.01-0.87, >3 months: HR=0.19, 95%CI 0.04-0.95), there was an interaction between anticoagulation duration and dose ( P=0.007). Conclusions:Based on the risk of thrombosis recurrence and bleeding events, the standard dose of rivaroxaban (20 mg qd) is recommended for patients with isolated distal deep vein thrombosis, and the anticoagulant duration should be maintained for 1.5 months or more.

ABSTRACT

@#Objective    To analyze the results and rationality of the lesion-focused strategy with subsegment as the pulmonary anatomical unit for pulmonary nodules with a diameter of ≤2 cm which require surgery. Methods    Clinical data of 246 patients with pulmonary nodules who underwent surgery in the Department of Thoracic Surgery of The First Affiliated Hospital of Nanjing Medical University from January 2017 to October 2018 were retrospectively analyzed, including 76 males and 170 females, with an average age of 53.30±11.82 years. The patients were divided into four groups, a single segmentectomy group, a segmentectomy combined with adjacent subsegmentectomy group, a single subsegmentectomy group and a combined subsegmentectomy group, according to the different surgical approaches, to compare preoperative, intraoperative, and postoperative related data. Results    There was no perioperative death. Among the four groups, there was no statistical difference in gender (P=0.163), age (P=0.691), diameter of the nodule (P=0.743), longitudinal position of the nodule (depth ratio, P=0.831), postoperative pulmonary leakage (P=0.752), intraoperative blood loss (P=0.135), pathological type (P=0.951) or TNM stage (P=0.995); there were statistical differences in transverse position of the nodule (P<0.001) and number of subsegments involved (P<0.001). The results of multivariate logistic regression analysis showed that compared with combined subsegmentectomy, the odds ratio (OR) of the lung nodule in segmentectomy combined with adjacent subsegmentectomy as intersegment nodules was 5.759 (95%CI 1.162 to 28.539, P=0.032). Conclusion     The surgical strategy of lesion  focused and subsegment as anatomical unit is safe and feasible for surgical treatment of pulmonary nodules with diameter ≤2 cm. The transverse position of the nodules is an important factor affecting the choice of surgical method for the middle and lateral nodules with a diameter of ≤2 cm, and the longitudinal location of the nodule is not an influencing factor. For nodules in inner zone, the diameter also is one of the factors influencing the choice of surgical method.

ABSTRACT

OBJECTIVE To explore the difference in th e mechanis m of baicalein and wogonin inhibiting the energy metabolism of hepatoma cells. METHODS Human hepatoma HepG 2 cells were divided into blank control group (without medicine),different dose groups of baicalein and wogonin (1.25,2.5,5,10 and 20 μmol/L). The effects of baicalein and wogonin on the viability of HepG 2 cells were detected by MTT assay. HepG 2 cells were divided into blank control group (without medicine),baicalein group and wogonin group. After administration ,the concentration of ATP in cell was detected by enhanced ATP kit. The levels of cell glycolysis and mitochondrial energy metabolism were evaluated by glycolysis and mitochondrial pressure test kit ;the affinity of baicalein and wogonin with key enzymes of energy metabolism was predicted by molecular docking ,and the key enzymes of energy metabolism with high affinity were screened ;the expression of key enzymes of energy metabolism was detected by Western blot. RESULTS Within the dose range of 2.5-20 μmol/L,the half inhibitory concentrations of baicalein and wogonin were 12.84 and 24.09 μmol/L;baicalein 1.25 μmol/L and wogonin 2.5 μmol/L had no effect on cell viability ,so it was selected as the dosage for subsequent experiments. Compared with blank control group ,the concentration of ATP in HepG 2 cells decreased significantly in baicalein group and wogonin group (P<0.05);the inhibitory effects on basic acidification rate of HepG 2 cells in wogonin group were significantly stronger than those of baicalein group (P<0.05),but there was no significant difference between them on the basic oxygen consumption rate (P>0.05);baicalein had strong binding to pyruvate kinase M 2 and mitochondrial enzyme complexes Ⅰ(CⅠ),C Ⅱ and C Ⅳ,while wogonin only had strong binding to pyruvate kinase M 2; wogonin could significantly down-regulate the protein expressions of hexokinase ,phosphofructokinase,pyruvate kinase M 2,CⅠ, C Ⅱ and C Ⅳ(P<0.05),but there was no statistical significance in the effect of baicalein on the regulation of these enzymes (P> 0.05). CONCLUSIONS Both baicalein and wogonin can inhibit the energy metabolism of hepatoma HepG 2 cells,but the mechanism is different :the effect of baicalein is related to the activity of key enzymes ,while the effect of wogonin is related to the inhibition of the expression of key enzymes of energy metabolism.

ABSTRACT

Previous studies believe that patent paraumbilical vein in cirrhotic portal hypertension can reduce portal venous flow, portal venous pressure, and the development of esophageal varices and esophageal variceal bleeding, but there are still controversies over this issue in clinical practice. This article reviews the formation of portal systemic collateral circulation, the characteristics of the paraumbilical vein, the definition and diagnosis of patent paraumbilical vein, and the influence of patent paraumbilical vein on the development of esophageal varices and esophageal variceal bleeding, and it is believed that patent paraumbilical vein may not reduce the development of esophageal varices and esophageal variceal bleeding. Contrary to the previous points of view, patent paraumbilical vein should be regarded as a manifestation of the progression of cirrhotic portal hypertension, which can lead to the complications such as hepatic encephalopathy, and therefore, targeted prevention measures should be adopted in clinical practice.

ABSTRACT

Cancer-associated fibroblasts (CAFs) and tumor-infiltrating immune cells are the most essential components of the tumor microenvironment (TME). They communicate with each other in tumor microenvironment and play a critical role in tumorigenesis and development. CAFs are very heterogeneous and different subtypes of CAFs display different functions. At the same time, it can contribute to the regulation of the function of tumor-infiltrating immune cells and eventually result in the carcinogenesis, tumor progression, invasion, metastasis and other biological behaviors of tumors by producting various growth factors and cytokines etc. Based on the current research results at home and abroad, this paper reviews the recent research progress on the regulation of CAFs on infiltrating immune cells in tumor microenvironment.

ABSTRACT

@#The development of osteoinductive bone-filling biomaterials for bone regeneration is of great significance.Self-assembled peptide hydrogels with high biomimetic extracellular matrix structure, low immunogenicity, easy synthesis and modification, and flexible loading capacity provide a highly efficient therapeutic platform for bone tissue repair.Herein, we discuss the design principles of self-assembled peptide hydrogels, report the structural characteristics and assembly mechanisms of self-assembled peptides, and highlight recent advances in self-assembled peptide hydrogels for bone regeneration, including delivery to cells , bone morphogenetic proteins, active factors and small molecular substances.Finally, the bottleneck and development direction of self-assembled peptide hydrogels are pointed out, aiming to provide guidance for the construction of hydrogel delivery systems with high osteogenic properties.

@#Polydopamine (PDA) nanoparticles were prepared as a carrier, and bavachinin (BVA) was efficiently loaded by physical adsorption.The erythrocyte membrane was further utilized to modify and construct the erythrocyte membrane biomimetic nanoparticles (RBC-BP), the residence time in the body was extended and the in vivo analytical method was established to investigate their pharmacokinetics in mice.Polydopamine nanoparticles loaded with BVA (BP) were prepared by solvent replacement method, and the influencing factors of PDA loaded with BVA were investigated with the adsorption rate as the evaluation index.The erythrocyte membrane was extracted and separated, and RBC-BP was prepared by incubation coextrusion method. The effects of pH value on membrane coating and the extrusion times on the particle size and uniformity of RBC-BP were investigated.The particle size, potential, morphology, and cumulative release rate of RBC-BP were systematically characterized, and their pharmacokinetics in mice were preliminarily explored.The results showed that the adsorption rate of BP was as high as (92.08 ± 0.17) % and the drug loading rate was (42.05 ± 2.95) % at the PDA to BVA ratio of 1∶0.5, the solution pH value of 7, the incubation time of 6 h, and the incubation temperature of 20 °C, and that the erythrocyte membrane could be successfully oriented and coated on the surface of BP by the action of electric charge at the pH value of 4. The in vitro studies showed that RBC-BP has the apparent core-shell structure with the particle size of (308.63 ± 6.56) nm and good stability, and in vivo pharmacokinetic studies showed that RBC-BP can significantly extend the circulation time of nanoparticles in vivo.

ABSTRACT

OBJECTIVE:To establish the method for the content de termination of 17 quality markers in Dahuang zhechong pills(DHZCP). METHODS :HPLC method was adopted to determine the contents of 17 quality markers in 10 batches of DHZCP , such as allantoin ,hypoxanthine,salidroside,hydroxypaeoniflorin,glycyrrhizin,isoglycyrrhizin,baicalin,p-methoxyphenylacetic acid,wogonin,cinnamic acid ,apigenin,naringin,norwogonin,aloe emodin ,rhein,chrysin,emodin. The determination was performed on Kromasil 100-5-C18(250 mm × 4.6 mm,5 μm)column with mobile phase consisted of 0.1% phosphoric acid solution-acetonitrile (gradient elution ) at the flow rate of 1.0 mL/min. The column temperature was 30 ℃ ,the detection wavelength was 210 nm and the sample size was 20 μ L. RESULTS:The linear range of above 17 quality markers were 5.74-183.53,6.51-208.24,4.30-137.65,4.60-147.06,4.12-131.76,4.25-135.88,6.31-201.76,4.60-147.06,1.94-62.06,4.47- 142.94,0.69-22.06,2.29-73.24,2.33-74.41,1.42-45.29,6.65-212.94,1.11-35.44 and 1.47-47.06 μg/mL,respectively(all R2≥ 0.999 0). RSDs of precision ,repeatability,stability and durability tests were all less than 2%(n=6);average recovery of 17 quality markers ranged from 96.31% to 101.73%,and the RSDs were less than 3%(n=6). CONCLUSIONS :The method is simple, rapid,speific,specise,reproducible,stable,accurate and durable ,and can be used for improving the quality standard of DHZCP.

ABSTRACT

Objective:To evaluate the feasibility and accuracy of a multi-dimensional peripheral quick contrast sensitivity function (pqCSF) measurement established based on Bayesian probability estimation algorithm.Methods:A cross-sectional study was conducted.Nineteen eyes of 12 healthy emmetropic subjects in Zhongshan Ophthalmic Center of Sun Yat-sen University from September 2017 to March 2018 were included, with an average age of (22.92±2.91) years.The average spherical power and cylindrical power were (-0.34±0.52)D and (-0.30±0.42)D, respectively, and the average uncorrected vision acuity was≥1.0.Based on the Bayesian probability algorithm, the peak contrast sensitivity γ max, the peak spatial frequency ? max, the bandwidth β and the low contrast intercept δ were used to quickly describe the contrast sensitivity function (CSF) curve of the full spatial frequency through multi-dimensional pqCSF method.The 16 peripheral visual field positions of all subjects were tested at 6°, 12°, 18° and 24° eccentricity of the superior, inferior, the temporal and nasal visual field by the pqCSF method, but the 18° eccentricity of temporal field, which was near the physiological blind spot, was excluded.The area under Log CSF (AULCSF) of different peripheral visual fields and the Log CSF of 19 spatial frequencies (distributed at equal intervals in logarithmic units) were compared.This study followed the Declaration of Helsinki, and the study protocol was approved by an Ethics Committee of Zhongshan Ophthalmic Center of Sun Yat-sen University (No.2018KYPJ017). Written informed consent was obtained from each subject prior to any examination. Results:With the increase of eccentricity in different visual fields, the AULCSF decreased gradually, and there were significant differences in AULCSF between different eccentricities (all at P<0.05). The AULCSF of the nasal and temporal visual field at 6°, 12° and 24° eccentricity was significantly larger than that of the superior and inferior visual field (all at P<0.05). As the distance from the fovea was increased, the pqCSF, the AULCSF, and the high-frequency cutoff were all decreased, and the standard deviation of AULCSF was increased gradually. Conclusions:The pqCSF method can depict a relatively complete peripheral CSF curve of a wide peripheral visual field, and reflect the function quality of the peripheral vision comprehensively and accurately.

ABSTRACT

Objective:To investigate the clinical value of pelvic floor biofeedback combined with acupuncture and moxibustion for the prevention of postoperative recurrence of endometriosis, and to evaluate its safety.Methods:From January 2017 to December 2017, 80 patients with endometriosis treated in Shenzhen Hospital of Guangzhou University of Traditional Chinese Medicine were selected and divided into observation group and control group according to the random digital table method, with 40 cases in each group.The control group received GnRH-A injection (zoladex) for 3 months after surgery.The observation group was treated with pelvic floor biofeedback combined with acupuncture on the basis of the control group.The cumulative recurrence of the two groups at 3, 6 and 12 months after operation was compared, and the safety and quality of life(QOL) of the patients were evaluated.Results:The cumulative recurrence rate of the control group was 12.5%(5/40) at 12 months after surgery, which was slightly higher than that of the observation group[10.0%(4/40)], but the difference was not statistically significant (χ 2=0.125, P>0.05). The serum alanine aminotransferase level of the two groups at 3 months after operation was within the normal reference range, and the difference was not statistically significant ( P>0.05). There was statistically significant difference in QOL score between the two groups at 3 months after operation ( t=4.923, P<0.01). Conclusion:The postoperative recurrence rate of pelvic floor biofeedback combined with acupuncture for the prevention and treatment of endometriosis is not significantly different from that of the control group, and it is safe and reliable.Besides, it can make the body immune system well regulated, improve microcirculation, and promote the quality of life of patients.

ABSTRACT

Objective:To assess the association between visual-related quality of life (QOL) and the severity of quick contrast sensitivity function (qCSF) defects in patients with early cataract.Methods:A cross section design was performed.Ninety-two eyes of 46 patients with early cataract were enrolled in Zhongshan Ophthalmic Center from March to June in 2017.All subjects completed the Chinese version of the National Eye Institute Visual Function Questionnaire (NEI VFQ-25). The qCSF measurements were performed monocularly and binocularly while the patients wearing full spectacle correction spectacles.Rasch analysis was performed to obtain final NEI VFQ-25 scores.Best corrected visual acuity (BCVA)(LogMAR) was measured and the eye with the better or the worse visual acuity was defined accordingly.The correlation between the Rasch calibrated QOL composite scores and the qCSF measurements of the better eye, the worse eye and binoculus, including the area under Log contrast sensitivity function (AULCSF), qCSF under cut-off frequency and 1.0, 1.5, 3.0, 6.0, 12.0, 18.0 c/d were analyzed.This study protocol was approved by the Ethic Committee of Zhongshan Ophthalmic Center.This study complied with Declaration of Helsinki.Results:The Rasch calibrated QOL composite score was 64.13±4.76.Significant negative correlations were found between composite scores of QOL and the BCVA of both the better or worse eye(the better eye: r=-0.386, P=0.008; the worse eye: r=-0.413, P=0.004). Significant positive correlations were found between the composite scores of QOL and the AULCSF of binoculus, the better and worse eye (binoculus: r=0.531, P<0.001; the better eye: r=0.524, P<0.001; the worse eye: r=0.711, P<0.001); Significant positive correlations were found between the composite scores of QOL and the qCSF under cut-off of binoculus, the better and worse eye (binoculus: r=0.504; the better eye: r=0.419; the worse eye: r=0.694, P<0.01). The composite scores of QOL was positively correlated with the qCSF under 1.5, 3.0, 6.0, 12.0 c/d of binoculus, the better and worse eye (binoculus: r=0.444, 0.544, 0.545, 0.493; all at P<0.05; the better eye: r=0.374, 0.506, 0.530, 0.431; all at P<0.05; the worse eye: r=0.513, 0.679, 0.710, 0.558; all at P<0.05). The composite scores of QOL was positively correlated with the qCSF under 1.0, 18.0 c/d of binoculus and the worse eye (1.0 c/d: r=0.296, 0.292; both at P<0.05; 18.0 c/d: r=0.386, 0.321; both at P<0.05). Conclusions:The qCSF measurements can reflect visual function of patients with early cataract at different spatial frequencies.The qCSF measurements of binoculus, the better eye and the worse eye show significant correlations with the composite scores of QOL.The QOL of early cataract patients shows a strong dependency on the worse eye.

ABSTRACT

Objective@#To investigate the effects of single-chamber balloon combined with dinoprostone suppository on the progestation of the cervix in full-term maternal delivery.@*Methods@#From January 2017 to December 2018, 120 cases of full-term maternal women admitted to the Central Hospital of Yiwu were selected.According to the random number table method, all women were divided into control group (60 cases) and observation group (60 cases). The control group was given a single-cavity balloon to promote cervical ripening.The observation group received single-chamber balloon combined with dinoprostone to promote cervical ripening.The effects of maternal cervical ripening were observed.@*Results@#The total effective rate of maternal cervical ripening in the observation group (95.00%) was significantly higher than that in the control group (81.67%), and the difference was statistically significant (χ2=6.135, P<0.05). The differences of maternal postpartum hemorrhage and neonatal asphyxia rate were not statistically significant (t=1.684 , χ2=2.563, all P>0.05). The total maternal labor [(5.8±2.1)h] in the observation group was significantly shorter than that in the control group [(7.6±2.5)h], the difference was statistically significant (t=3.126, P<0.05). The vaginal delivery rate (95.00%) of the observation group was higher than that of the control group (73.33%), the difference was statistically significant (χ2=5.342, P<0.05). Before implanting the balloon and balloon combination, the Bishop score between the two groups had no statistically significant difference (t=3.623, P>0.05). After the balloon and balloon combination were taken out, the Bishop score of the maternal cervix[(8.6±1.5)points] in the observation group was higher than that in the control group [(7.2±1.3)points], and the difference was statistically significant (t=4.354, P<0.05). The incidence of adverse reaction of the observation group (6.67%) was lower than that of the control group (13.33%), the difference was statistically significant between the groups(χ2=3.821, P<0.05).@*Conclusion@#Compared with simple single-chamber balloon, single-chamber balloon combined with dinoprostone suppository has more significant effect on the maternal ripening of the cervix, it can improve the vaginal delivery rate and maternal cervical Bishop scores, effectively reduce the maternal total labor, and reduce the incidence of maternal adverse reactions.

ABSTRACT

Due to limited availability of autologous blood vessels (blood vessels from the same recipient used for vascular transplantation materials) and inadequate growth ability of non-autologous blood vessels (artificial blood vessel transplantation materials), more and more attention has been paid to tissue engineering blood vessels. In this study, we constructed an ammonium phosphate zwitterion modified acellular vascular scaffold with highly biocompatible bone marrow-derived endothelial progenitor cells as the inner layer of a new vascular transplantation material. The vascular acellular scaffolds were modified by a simple method-co-precipitation method. The platelet adhesion test, hemolysis test, recalcification test and cytotoxicity of acellular vascular scaffolds in vitro were evaluated. Ammonium phosphate zwitterions modified endothelial progenitor cells on the surface of acellular scaffolds with concave and convex structure on the surface of natural vascular lumen can be effectively promoted by improving anticoagulant activity. Modified acellular scaffolds have similar mechanical properties to natural blood vessels and can effectively construct endothelialization in vitro. The results of this study provide a preliminary exploration for the modification of vascular acellular scaffolds to achieve anti-thrombosis and endothelialization in vitro.

Objective To investigate the effects of single-chamber balloon combined with dinoprostone suppository on the progestation of the cervix in full-term maternal delivery.Methods From January 2017 to Decem-ber 2018,120 cases of full-term maternal women admitted to the Central Hospital of Yiwu were selected.According to the random number table method ,all women were divided into control group (60 cases ) and observation group (60 cases).The control group was given a single-cavity balloon to promote cervical ripening.The observation group received single-chamber balloon combined with dinoprostone to promote cervical ripening .The effects of maternal cervical ripening were observed.Results The total effective rate of maternal cervical ripening in the observation group (95.00%) was significantly higher than that in the control group (81.67%),and the difference was statisti-cally significant (χ2 =6.135,P<0.05).The differences of maternal postpartum hemorrhage and neonatal asphyxia rate were not statistically significant ( t =1.684 ,χ2 =2.563,all P>0.05).The total maternal labor [( 5.8 ± 2.1)h] in the observation group was significantly shorter than that in the control group [(7.6 ±2.5) h],the differ-ence was statistically significant (t=3.126,P<0.05).The vaginal delivery rate (95.00%) of the observation group was higher than that of the control group (73.33%),the difference was statistically significant ( χ2 =5.342,P<0.05).Before implanting the balloon and balloon combination ,the Bishop score between the two groups had no statis-tically significant difference (t=3.623,P>0.05).After the balloon and balloon combination were taken out ,the Bishop score of the maternal cervix [(8.6 ±1.5)points] in the observation group was higher than that in the control group [(7.2 ±1.3)points],and the difference was statistically significant (t=4.354,P<0.05).The incidence of adverse reaction of the observation group (6.67%) was lower than that of the control group (13.33%),the differ-ence was statistically significant between the groups ( χ2 =3.821,P<0.05).Conclusion Compared with simple single-chamber balloon,single-chamber balloon combined with dinoprostone suppository has more significant effect on the maternal ripening of the cervix ,it can improve the vaginal delivery rate and maternal cervical Bishop scores , effectively reduce the maternal total labor ,and reduce the incidence of maternal adverse reactions.

ABSTRACT

Objective To explore the relationship between the rs18004197 polymorphism in the 3'-untranslated region of adenomatous polyposis coli (APC) gene and colorectal cancer susceptibility.Methods Firstly,we collected the peripheral venous blood of 573 colorectal cancer cases and 588 controls,and then extracted DNA from blood samples,genotyped rs1804197 polymorphism using real-time PCR and assessed its association with the susceptibility of colorectal cancer.Results There were 387 CC (67.5%),153 AC (26.7%) and 33 AA (5.8%) genotypes in the colorectal cancer cases.In the control group,there were 427 CC (72.6%),144 AC (24.5%) and 17 AA (2.9%) genotypes.The AA genotype odds ratio (OR =2.14,95% CI:1.17-3.91,P =0.011) and the A allele frequency (P =0.011) were significant difference in case and control groups.Further subgroup analysis showed that the differences of the frequency distribution in the male (P =0.048) and non-drinking (P =0.020) groups were statistically significant.In the male group,the risk of colorectal cancer was increased by 0.41 (OR =1.41,95% CI:1.01-1.98) for individuals bearing the A allele.In the non-drinking group,the risk of colorectal cancer was increased by 0.22 (OR =1.22,95% CI:0.91-1.64) for individuals bearing the A allele,but the result was not statistically significant.Conclusion The rs18004197 polymorphism in the 3'-untranslated region of APC gene is related to the susceptibility of colorectal cancer.The AA genotype may increase the susceptibility of colorectal cancer.

ABSTRACT

The aim of the present study was to increase distribution of yuanhuacine in the lungs and achieve the purpose of reducing toxicity and increasing efficiency.Therefore,yuanhuacine was designed to be dry powder inhalers innovatively and directly delivered to the lungs.Accordingly,inhaled lactose was used as a carrier to adsorb yuanhuacine on the surface of lactose.Fine particle fraction (FPF) was utilized as evaluation index to filtrate the optimal prescription for pulmonary administration.Besides,an UHPLC-MS/MS method was established for the analysis of heart,liver,spleen,lung,kidney,brain and reproductive system of rats.Intravenous injection was taken as reference to investigate the distribution of yuanhuacine and calculate relevant targeting parameters.The experimental result indicated that the prescription (rough lactose ∶fine lactose =10 ∶ 1) has the highest FPF,which can be chosen as the most suitable prescription for pulmonary administration of yuanhuacine.Moreover,by comparing the distribution of yuanhuacine through pulmonary administration and intravenous injection,it was found that the concentration of yuanhuacine in the lung tissue was greatly increased by pulmonary administration,which decreased the distribution in heart,liver,spleen,kidney,brain and reproductive system,thus sequentially reducing the toxicity in other tissues and increased the efficiency.

ABSTRACT

OBJECTIVE@#To investigate the application value of narrow band imaging (NBI) in early diagnosis of pharyngolaryngeal tumors.@*METHOD@#A total of 106 patients received NBI endoscopy in the endoscopic diagnosis. Lesions found under the white-light endoscopy mode and NBI endoscopy mode were compared in the morphology of capillaries on lesiorn surface and the clarity of lesion rim. Biopsy was performed in suspected areas with those two endoscopies for the lesions found under white-light endoscopy and NBI endoscopy, the morphology of capillaries on the surface of lesion and the clarity of lesion boundary were compared between both. Biopsy was performed for suspected areas under two modes, and specimens were preserved in 10% formaldehyde for pathological examination. The characteristic, position and endoscopic diagnosis under two modes were recorded. All the patients underwent corresponding laryngeal tumor resection according to the histopathological result of biopsy, and the histopathological result of resected tissues was taken as the gold standard for diagnosis. The biopsy detection rate and biopsy correct detection rate of malignant lesions in two groups were calculated and statistical compared.@*RESULT@#The diagnostic accuracy under white-light mode was 75.47%, while that under NBI mode was 96.23%, and the difference between them was statistically significant (χ² = 18.375, P < 0.01). The biopsy correct detection rate under white-light mode was 82.08%, while that under NBI mode was 95.28%, and the difference between them was statistically significant (χ² = 12.071, P < 0.01). The correct detection rate of malignant tumor under white-light mode was 48.15%, while that under NBI mode was 92.59%, and the difference between them was statistically significant (χ² = 10.083, P < 0.01).@*CONCLUSION@#Using NBI endoscopy to observe the morphological changes of capillaries on the pharyngolaryneal mucosa surface can increase the detection rate of early pharyngolaryngeal tumors, so it is worth to be widely applied.

ABSTRACT

OBJECTIVE@#To understand the vowel duration and statement reading of the adductor spasmodic dysphonia (ADSD) patients compared with their normal controls, and provide ideas for clinical diagnosis and treatment.@*METHOD@#Twenty-nine ADSD patients were included in the research, with 31 normal controls. All subjects filled in form voice handicap index (VHI) by themselves. Maximum phonetic time (MPT) and maximum loudness phonetic time(MLPT) were tested on /a/ sound for all patients. Also, all the patients were required to read aloud a standard mandarin assay named , duration were measured with Praat5. 0 software after sounds were collected. A one-way t-test was performed to compare spasmodic group with control group on VHI, MPT, MLPT and duration for reading standard sentences. Pearson/Spearman correlation was tested. Result: The VHI of the 29 ADSD patients is 89±12, and their normal controls 15±16, indicating that the VHI in ADSD group is significantly higher than in the control group(P<0. 01). The MPT of the ADSD group is(16. 9±9. 8 s), and the control group is (25. 3±10.0)s, indicating that MPT in the ADSD group is significantly shorter than the control group(P<0. 01). The MLPT of the ADSD group is (15.7±7. 6)s, and the control group is (26. 5±11. 4)s, indicating that MLPT in the ADSD group is significantly shorter than the control group (P<0. 01). The duration of standard sentence reading of the ADSD group is (55.0±14. 2)s, and the control group is (37. 8±4. 8)s, indicating that the duration of standard sentence reading in the ADSD group is significantly longer than the control group (P<0. 01). Correlation analysis showed that MPT and MLPT are related within the ADSD group(r=0. 697,P< 0.01), other indexes being tested have no significant correlations.@*CONCLUSION@#The voice disorder condition of the ADSD patients is significantly worse than normal people. Their pronunciations on continuous vowels are not lasting compared with normal people. In the meantime, their ability to read sentences is worse than normal people, needing longer time to complete the reading task. There is a significant correlation between MLPT and MPT, which can be used as an alternative to MPT.

OBJECTIVE@#To investigate the characteristics of adductor spasmodic dysphonia phonatory break in mandarin Chinese and select the stimuli phrases.@*METHOD@#Thirty-eight patients with adductor spasmodic dysphonia were involved in this study. Standard phrase " fù mŭ xīn" and a speech corpus in mandarin Chinese with 229 syllables covering all vowel and constant of mandarin Chinese were selected. Every patient read the phrases above twice in normal speed and comfortable voice. Two auditory perpetual speech pathologists marked phonatory break syllables respectively. The frequency of phonatory break syllables and their located phrases were calculated, rated and described. The phrases including the most phonatory break syllables were selected as stimuli phrases, the phonatory break frequency of which was also higher than that of standard phrase "fù mŭ xīn".@*RESULT@#Phonatory break happened in the reading of all patients. The average number of phonatory break syllables was 14 (3-33). Phonatroy break occurred when saying 177 (77.3%) syllables in the speech corpus. The syllables "guŏ, rén, zāng, diàn, chē, gè, guăn, a, bā, ne, de" broke in 23.1%-41.0% patients. These syllables belonged to the phrases "pĭng guŏ, huŏ chē, shì de, nĭ shì gè hăo rén, wŏ mén shì yŏu zŏng shì bă qĭn shì nong dé hĕn zāng, wŏ mén nà biān yŏu wăng qiú yùn dong chăng, cān gŭan, jiŭ bā hé yī gè miàn bāo dìan, tā shì duō me kāng kăi a,wŏ yīng gāi zài xìn lĭ xiĕ yī xiē shén mē ne?". Thirty-seven patients (97.3%) had phonatory break in above mentioned words. Ratios of these words phonatory break also were more than "fù mŭ xīn".@*CONCLUSION@#Adductor spasmodic dysphonic patients exhibited different degrees of phonatory break in mandarine Chinese. The phrases" shì de, pĭng guŏ, huŏ chē, nĭ shì gè hăo rén, wŏ mén nà biān yŏu wăng qiú yùn dong chăng, cān gŭan, jiŭ bā hé yī gè miàn bāo dìan, tā shì duō me kāng kăi a" were recommended as stimuli phrases for adductor spasmodic dysphonia evaluation.
